Grounded AI answers questions from a defined, trusted body of content rather than from a model’s general training — retrieval-augmented generation (RAG) retrieves the relevant validated material first, then generates an answer from it, with citations back to source. In operations, grounding is the difference between an answer and a liability.

What it is
A general model asked an operational question will answer fluently from everything it has ever read — including nothing about your operation. RAG inverts the order: retrieve the relevant approved content first, generate only from what was retrieved, cite what was used. The answer’s quality becomes a function of the knowledge base’s quality — which is why grounding without governance only relocates the problem: retrieving from a stale wiki grounds you in stale answers.

Operational questions have wrong answers with consequences.
